Job Opportunity: Community Nursing Support Worker
Do you have experience of working with Older Adults with Mental Health issues related to cognitive impairment and Dementia?
A rare opportunity has become available to join our new Dementia Intensive Support Team covering CTM as a community nursing support worker.
About the Role
The DIST support people to live in their own homes and to live well with dementia.
To work as a member of the Dementia Intensive Support Team. To assist in the implementation of care for service users and their families, who are experiencing a deterioration of signs/symptoms of Dementia and require intensive home treatment as an alternative to inpatient care. To deliver an effective and competent level of care, under the guidance and supervision of the multi-disciplinary team.
- Undertake care or supports self-care as directed by the registered health-care professional, following agreed care/recovery plans, providing information to the registered healthcare professional that will enable the monitoring and amendment of care/recovery plans and ongoing risk assessment. For example, this may include working closely with patients/ service users to meet their individual needs in achieving independence, supporting with life skills, resuming integration into social settings, actively helping participation in activities to facilitate recovery and achieve their goals.
- Develop rapport based upon a therapeutic working relationship, encouraging and motivating engagement in the individual care/ recovery plan, maintaining appropriate boundaries at all times.
- Observe patients for signs of agitation or distress, and record accurate and timely information in the appropriate documentation including documenting for example indicators of positive recovery, patient/service user strengths, presence of relapse indicators.
