As an Obesity Care Co-ordinator

You have experience in a healthcare setting and will be an integral part of our Tier 3 Weight Management team, working in close partnership with the Consultant Endocrinologist to provide coordinated, patient-facing support.

Essential:

  • Experience in a healthcare setting, ideally within patient services, specialist clinics, or clinical administration
  • Confidence working alongside senior clinicians, including consultants, in a fast-paced specialist setting
  • Strong ability to handle sensitive data and a great attention to detail
  • Good understanding of Information Governance procedures to ensure the security of patient data
  • Excellent verbal and written communication skills
  • Competent user of IT equipment and essential software (email, word processing, data entry and interpretation)
  • Experience in clinical coding, with the ability to accurately assign and interpret clinical codes (e.g. SNOMED CT, ICD-10) in line with NHS standards
  • Ability to thrive in challenging, fast-paced situations
  • Highly organised with the ability to work autonomously
  • Ability to prioritise your workload based on business needs
  • Passion for providing digital treatment & support
  • Excellent problem-solving and critical-thinking skills with the ability to manage complex patient cases

Desirable:

  • Previous EMIS experience
  • Experience of managing a large patient caseload in a specialist or secondary care setting
  • Experience of managing safeguarding scenarios
  • Familiarity with endocrinology or obesity medicine pathways

Key Responsibilities:

  • Delivering high-quality specialist care coordination to patients enrolled in our Tier 3 Weight Management programme, under the clinical oversight of the Consultant Endocrinologist
  • Manage and co-ordinate the care of patients being treated with novel GLP-1 RA obesity therapies, ensuring timely follow-up and escalation to the Consultant where clinically indicated
  • Act as the day-to-day patient-facing link between patients and the Consultant Endocrinologist, facilitating communication and continuity of care
  • Organise and manage the Consultant Endocrinologist's appointment schedule, ensuring efficient use of clinical time and a smooth experience for patients
  • Ensure key treatment and outcome data is kept up to date for treatment and audit purposes
  • Work with other team members to develop and refine efficient patient systems and workflows
  • Work closely with the Patient and Clinical Services teams to ensure a smooth patient journey from onboarding to discharge
  • Act as a point of contact for all administrative support within the Consultant Endocrinologist's team
