Job Description
This is a fixed term post to support the ICSS team through times of high demand. ICSS is a supportive and friendly MDT providing assessment and rehabilitation for people in their homes and local community. Supervision would be provided by stroke specialist clinicians and you would have access to inservice training and advice from clinicians in the team. This is a great opportunity to develop your skills in stroke rehabilitation and occupational therapy.
Purpose of the Role
- To provide specialist assessment and rehabilitation for people with stroke in their homes and local community.
- To work with the MDT to support people's transfer of care from hospital to home enabling flow from the hospital setting.
- To work closely with other professions to provide holistic care to patients and their families.
- To provide informal supervision for rehabilitation support workers and students.
- To set up and monitor treatment plans for personal care, meal preparation and therapy programmes for support staff to follow.
- Collaborate and negotiate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan, which will maximise quality of life and independence.
- Facilitate appropriate referrals to other services or investigations based on your assessment to ensure holistic patient care.
- To assess and arrange the provision of equipment and/or adaptations from appropriate agencies ensuring this is provided in good working order and fitted correctly; reporting any issues appropriately.
