Occupational Therapist
We are seeking an Occupational Therapist to join our multidisciplinary Community Rehabilitation Team. The post will suit an enthusiastic Occupational Therapist with an interest in developing skills across a wide range of therapeutic approaches.
About the Role
Our Community Rehabilitation Team supports individuals with complex and enduring mental health needs to achieve recovery and independence within the community across the Pennine Care footprint. We are committed to delivering high-quality, evidence-based assessment and treatment, psychological care and fostering a culture of innovation, compassion, and collaboration.

Responsibilities
- Embed the Occupational Therapy assessment and intervention pathway across the Community Rehabilitation Team, within the framework of Model of Human Occupation in collaboration with the wider MDT.
- Be responsible for a caseload of service users under the care of the Community Rehabilitation Team, supporting them in their recovery journey.
- Undertake specialist OT assessments in all areas of self-care, productivity and leisure to support discharge planning and identify support needs.
- Conduct OT assessments, developing outcome-based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence-based outcome measures.
- Provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members.
- Develop treatment plans and intervention to be delivered by the team’s social inclusion workers.
- Provide clinical supervision to the junior staff within the Community Rehabilitation Team.
