Band 6 Occupational Therapist
We are looking to hire a Band 6 Occupational Therapist to join our team. The successful candidate will provide occupational therapy assessments and treatment services for clients admitted to the older adult inpatient ward in Hammersmith and Fulham Mental Health Unit.
Role Overview
The role is based in Meridian Ward.
Responsibilities
- To conduct comprehensive functional assessments for patients within the locality who are experiencing acute mental health diagnosis and cognitive impairment. All assessments and interventions are predominantly conducted in the patient’s home environment.
- To conduct assessments to support patients who experience deterioration in their physical health and provide interventions to support their functioning.
- To provide specialist occupational therapy to clients with complex needs using evidence-based / client-centered principles to assess, plan, implement, and evaluate interventions at all stages of the service user’s journey; cognitive and functional assessment to support diagnosis, advising on dementia design, home modification, assistive technology, therapeutic activities, and cognitive rehabilitation.
- To conduct comprehensive assessments and evidence-based interventions utilizing the occupational therapy process. Deliver personalized, and goal-orientated rehabilitation to enable timely discharges, reduce care package, and delay care home placements.
- To actively develop and implement a framework of care, which reflects a comprehensive approach to service user needs, and the seven pillars of clinical governance.
- To use evidence-based principles to apply clinical skills and reasoning in occupational therapy, assessment, treatment planning, intervention, and evaluation which include individual and group-oriented approaches.
