To work within the Home First pathway with a view to ensuring that patients remain in their home environment.

  • To work in the community with patients who have long term conditions and/or palliative care needs.
  • To assess and treat patients whilst following the Occupational Therapy process.
  • To develop and deliver specialist Occupational Therapy for people with long term conditions and palliative care needs within the community.
  • To work within the Doncaster Community Therapy Service to deliver the Home First model to people in the Doncaster area, to prevent hospital admissions and accelerate hospital discharges.
  • To assess individual patient’s needs, set and implement treatment plans and long term condition management for patients linking with primary, secondary and tertiary care, using a coordinated approach.
  • To contribute to the development of the Occupational Therapy role within the Occupational Therapy Service by working across boundaries and inter agency working.
  • As part of a team, initiate research based practice and monitor the quality of the service being provided.
