Are you an Occupational Therapist looking for a rewarding community-based role where you can help adults build independence, achieve meaningful goals and remain safe in their own homes?

We are recruiting for a Rehabilitation Occupational Therapist to join a supportive Adult Social Care team. You will work with adults with disabilities and rehabilitation needs, delivering practical, person-centred interventions that reduce reliance on ongoing care and support better everyday outcomes.

The role

You will complete home-based assessments to understand an individuals needs, risks and rehabilitation potential. Working alongside the person and relevant professionals, you will develop practical rehabilitation plans and goals, recommend suitable equipment, minor adaptations and daily living techniques, and review progress throughout their support. You will also maintain accurate case records, contribute to case triage and offer Occupational Therapy guidance to colleagues. You'll also need experience with moving and handling along with reviewing packages of care.

What you'll need

  • Degree in Occupational Therapy, or due to qualify shortly
  • Experience of working with vulnerable adults and supporting daily living activities
  • A strong understanding of rehabilitation and how Occupational Therapy improves independence
