Working as a senior member of an established Community Mental Health Team in the North Locality of Doncaster providing a high standard of safe, robust and evidence-based clinical practice.
- Working with complex organic and functional diagnosis within the community setting, liaising and integrating with both statutory and voluntary organisations in support of the specific client group.
- To promote and maintain the highest attainable standard of nursing care in a community setting.
- To support the band 6 nursing team with the implementation of evidence-based interventions from a plan of care.
- Perform re-assessments of clients to include risk and crisis plans.
- To maintain close working relationships with referring agencies.
- To deputise for the Team Leader when required.
- To carry a caseload of patients with both functional and organic issues as identified through the team systems process.
