Hospice Out of Hours Nurse - Bank/ Casual

(Role equivalent to Agenda for Change Band 5)

Payment

  • Standard on-call payment per shift: £100
  • Initial call out: Additional £60 payment
  • Any further call outs: Additional £20 per call out
  • 8pm cancellation: 4 hours pay will be due (£40)
  • Premiums: Payable in relation to the standard on-call payment (£100 element only) for Saturdays, Sundays and Evora Hospice recognized bank holidays
  • No payment for any shift cancelled due to no patients or your non-attendance

Hours

  • On call from 22.00 to 08.00 on any shift

Location

  • Community based role delivering care in patients’ homes throughout the catchment area (BT35 greater Newry City)

  • Departing from own home

  • Community Services Manager

Main Purpose of Job

Working as part of the Hospice Out of Hours Nurse Team, to be responsible for providing a nursing service to patients, who have a cancer diagnosis and are at end-of-life, as well as their families. To participate in the assessment, planning, implementing, and evaluating of care of patients in their own home encompassing the Hospice’s philosophy of palliative care. To collaboratively work with GPs, SHSCT District Nursing Team, Out of Hours, Marie Curie Service and SHSCT Specialist Palliative Care Team, to ensure effective communication in the provision of care for patients and their families at the end of life.

Main Responsibilities

Clinical Responsibilities

  • Provide nursing care to meet patient’s needs and to enable symptoms to be well controlled at end of life
  • Maintain clinical competencies as defined by the needs of the service such as management of syringe drivers, administration of oral medication, administration of intravenous drug therapies, wound management, stoma care and basic life support
  • Work effectively with community professionals, hospital, and community palliative care teams, ensuring effective communication between all service providers
  • Build and maintain trusting relationships with patients, and their families/carers demonstrating empathy, dignity and understanding at all times
  • Ensure safe handling and administration of medicinal products in accordance with Hospice procedures, NMC requirements and RQIA standards
  • Provide advice and support for the carers and family according to their needs and the level of intervention required at end of life
  • Maintain excellent communication with patients, relatives and carers ensuring that the patient’s individual needs are met, reporting any issues or concerns, including safeguarding risks, to the Community Services Manager
  • Regularly work without supervision for all of your shift
  • Participate in the orientation, induction, and training of new staff and support, supervise and mentor staff
  • Refer any concerns around poor performance and practice to the Community Services Manager
  • Assist in maintaining own and others health, safety and security

General Responsibilities

  • Take responsibility for managing and prioritising own workload in conjunction with the Community Services Manager
  • Work independently, as part of the Hospice Out of Hours Nursing Team and also as part of the wider Hospice team
  • Promote and be an advocate for Evora Hospice at all times

Professional Responsibilities

  • Maintain professional registration with the NMC, adhering to the NMC code at all times
  • Ensure professional and personal development and accountability in accordance with the NMC Revalidation requirements, ensuring revalidation is completed 3 yearly
  • Practice in accordance with Evora Hospice policies and procedures
  • Actively participate in the annual performance review process to identify opportunities for learning and development
  • Participate in continual professional development as required, ensuring required training is kept up-to-date and renewed on a timely basis
  • Actively participate in clinical supervision and team meetings to ensure a contribution to the professional development of the nursing team
  • Maintain appropriate car insurance to cover business travel purposes and provide evidence of this at the time of annual renewal

Reporting, Records Evaluation

  • Maintain accurate records (manually/ electronically) as requested, ensuring all documentation is legible, accurate, written in black ink and clearly signed and time date in accordance with NMC standards
  • Adhere to all relevant Hospice policies, including Confidentiality and Data Protection (GDPR)
  • When working remotely, ensure sensitive information is treated with due care and respect to maintain confidentiality and security and that any Hospice property is stored out of sight in the boot of your car whilst away from Hospice premises (and that your car is locked when unattended)

Regulations Standards

  • Lone working is a feature of this job given care will be delivered within the patient’s own home – Evora Hospice’s Lone Working and Health and Safety Policies must be adhered to at all times
  • All work must be undertaken in adherence to the ethical standards of practice as set down by the Department of Health (NI), NMC and in accordance with RQIA requirements
  • All safeguarding measures must be adhered to in undertaking the duties of this role

Essential Criteria

  • Registered on part 1 of the NMC register
  • 2 years’ post registration experience
  • Knowledge in the administration of medicines
  • Ability to remain calm under pressure and be sensitive to various situations
  • Excellent record-keeping skills
  • Excellent communication and interpersonal skills
  • Ability to demonstrate initiative and be proactive
  • Good organizational and time management skills
  • Ability to work on own and as part of a team
  • Excellent accuracy and attention to detail
  • Enthusiastic with a self-motivated approach
  • Tact, patience and diplomacy and the ability to maintain a sensitive and professional approach towards colleagues and patients in a confidential setting
  • Full driving license with eligibility to drive in the UK and access to a car for work purposes
  • Must live within 50 minutes travelling time (measurable by Google Maps) of any address within the catchment area

Desirable Criteria

  • Experience in a palliative care setting or in caring for patients with a palliative care diagnosis
  • Experience of providing care in patients’ homes
