About the Role

As the Outreach Team Information and Support Coordinator (ISC), you’ll be at the heart of Oakfield Outreach, coordinating referrals, triaging requests for support and ensuring children, families and schools are connected to the most appropriate services quickly and effectively. You’ll oversee information systems, maintain accurate performance data and work closely with schools, families and partner agencies to support a coordinated, trauma-informed approach to inclusion.
Working alongside the Head of Secondary Outreach and the wider team, you’ll ensure referrals are managed efficiently, support multi-agency working, coordinate meetings and forums, and provide line management to Administration Assistants. Your work will help ensure services are responsive, consistent and focused on achieving positive outcomes for vulnerable children and young people.
Key Responsibilities
- Coordinate and triage referrals, ensuring timely responses and appropriate support pathways.
- Manage information systems, data quality and performance reporting to support effective decision-making and service improvement.
- Work collaboratively with schools, families and partner agencies to coordinate support and promote inclusive practice.
- Coordinate meetings, locality forums and multi-agency activity, ensuring effective communication and follow-up.
- Supervise and support Administration Assistants to ensure efficient and responsive service delivery.
- Maintain accurate records and ensure compliance with safeguarding, confidentiality, GDPR and local authority procedures.
- Support the continuous development of Oakfield Outreach services, promoting early intervention and reducing exclusion.

About You
We’re looking for an organised, proactive and collaborative professional who is committed to improving outcomes for children and young people.
- Qualifications plus experience or experience without a qualification i.e. BTEC Higher or NVQ 3 (or equivalent) + post qualification experience, the experience item must be retained or Substantial experience of working in children or family services within a SEND, health, social care or education context.
- Experience of triage, referral management, family support, advisory or case coordination work with vulnerable children and families.
- Experience of maintaining accurate records, management information systems and producing reports to support service improvement and outcomes monitoring.
- Experience of building successful collaborative relationships with schools, families, partners and external agencies that command trust, confidence and respect.
- Experience of coordinating meetings, forums or multi-agency activities including minute-taking and follow-up actions.
- Excellent organisational skills with the ability to manage a high-volume, fast-paced workload and competing priorities.
- Competence in the use of digital systems, databases and Microsoft Office applications.
