Job Description: Band 6 Occupational Therapist
We are seeking to appoint an experienced, enthusiastic, motivated and forward thinking Band 6 occupational therapist to work within the Specialist Palliative Care service based at City Hospice. Experience of working in the community is desirable but not essential as the successful candidate will receive a full supportive induction when they join the team.

Responsibilities
The post holder will contribute to the provision of specialist palliative care, supporting patient choice in maintaining dignified end of life care, to the people of Cardiff with malignant or non-malignant conditions who have a limited life expectancy. The post holder will be working autonomously and as part of a team. They will deliver therapeutic interventions in patients’ homes, and/or when they attend City Hospice as an out-patient or attend our well-being services. They will provide advice, education and support to patients, their carers and other health care professionals. They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations, whilst meeting the integrated care needs of the patient.

Requirements
The post holder will also need to be passionate about delivering quality care to people receiving palliative or end of life care.
We would like to hear from any HCPC registered Band 6 occupational therapists with relevant post-graduation experience who have a desire to make a difference and enjoys the opportunity to care for people in their preferred place of care.
