Job Title: Paramedic Practitioner

Reports to: GP lead and Practice Manager

Hours: Maximum of full-time 37.5 hours per week

K&W Healthcare Ltd is a GP network providing a wide range of primary care services to a population of over 180,000 patients across 28 GP practices within Kingsbury, Willesden and Wembley. Our services include Clinical Pharmacy, Phlebotomy, Integrated Care Services and multidisciplinary patient care. We are committed to delivering high-quality, patient-centred healthcare and improving outcomes through collaborative working across Primary Care.

Job Summary:

We are seeking an experienced, HCPC-registered Community Paramedic Practitioner with previous experience working within a GP Practice or Primary Care setting.

The successful candidate will work autonomously as part of a multidisciplinary team, providing high-quality clinical assessment, diagnosis, treatment and ongoing management of patients presenting with acute, minor illness and long-term conditions.

The role includes face-to-face consultations, telephone triage, home visits and care home support. You will work closely with GPs, nurses and other healthcare professionals to deliver safe, effective and patient-centred care while helping reduce unnecessary hospital admissions and improving access to primary care services.

The post holder must demonstrate excellent communication skills, sound clinical judgement and experience working within GP Practice systems and processes.

Clinical Duties

  • Assess, diagnose, treat and manage patients presenting with undifferentiated and undiagnosed conditions.
  • Provide autonomous clinical assessment within agreed scope of practice.
  • Conduct same-day urgent appointments.
  • Undertake telephone and face-to-face consultations.
  • Carry out home visits and care home assessments.
  • Develop and implement patient-centred management plans.
  • Support the management of patients with long-term conditions.
  • Refer patients appropriately to secondary care and community services.
  • Work collaboratively with GPs and the wider multidisciplinary team.
  • Participate in clinical decision-making and case discussions.
  • Promote health education and self-management.
  • Prescribe medication where appropriately qualified.
  • Undertake venepuncture, point-of-care testing and specimen collection where competent.
  • Respond appropriately to medical emergencies.
  • Ensure accurate clinical documentation using GP clinical systems.
  • Participate in clinical audits and quality improvement initiatives.

Administrative Responsibilities

  • Maintain accurate, contemporaneous patient records.
  • Complete all clinical coding and documentation.
  • Participate in practice meetings and MDT meetings.
  • Maintain patient confidentiality at all times.
  • Ensure compliance with CQC standards, NHS policies and clinical governance.

Professional Responsibilities

  • Maintain HCPC registration.
  • Maintain professional indemnity.
  • Participate in Continuing Professional Development (CPD).
  • Attend mandatory and statutory training.
  • Participate in annual appraisal and revalidation.
  • Maintain evidence-based clinical practice.

Person Specification

Qualifications:

  • HCPC Registered Paramedic
  • Evidence of Continuing Professional Development

Essential experience:

  • Minimum two years' post-registration experience as a Paramedic
  • Previous experience working within a GP Practice or Primary Care setting
  • Experience using GP clinical systems (EMIS, SystmOne or similar)
  • Experience undertaking home visits
  • Experience managing acute and chronic conditions
  • Experience working within a multidisciplinary team
  • Experience of audit and quality improvement.

Desirable:

  • Independent Prescribing Qualification
  • Knowledge of NHS clinical governance
  • Knowledge of CQC standards

Other Requirements

  • Enhanced DBS Check.
  • Full UK Driving Licence.
