Role Summary

The Patient Follow-Up Coordinator is responsible for the day-to-day administration and coordination of Remote Monitoring and Supported Self-Management Pathways (SSMP) within the Cancer Support Team. The post holder will manage patient activity across remote monitoring systems, ensuring accurate data entry, monitoring pathway compliance, undertaking quality assurance checks, identifying and escalating data gaps, and tracking patients throughout their follow-up journey.

  • Work closely with Cancer Support Workers, Cancer Nursing teams, Prehabilitation services, and other multidisciplinary colleagues
  • Support the delivery of safe, effective, and patient-centred follow-up care for people living with and beyond cancer
  • Provide comprehensive administrative and project support to the Cancer Support Team, including coordinating meetings, maintaining records, supporting service improvement initiatives, and acting as a key point of contact for patients and healthcare professionals involved in remote monitoring pathways

Person Specification

Essential Criteria

  • Admin and Clerical experience
  • 2 years NHS experience
  • Excellent customer care skills to communicate with patients and relatives, in person, and by telephone managing barriers of communication
  • Ability to work as part of a team in a multidisciplinary environment
  • Ability to communicate effectively with all multi-disciplinary professionals across all departments, and with GP practices and other Trusts
  • Ability to act on own initiative within parameters set by service
  • Ability to prioritise, organise own workload, and to work to set deadlines
  • NVQ Level 3 or equivalent in Business/Admin
  • Flexible, open-minded, and proactive with identifying and making improvements
  • Strong focus on providing a high-quality and personable customer experience
  • Works effectively both autonomously under own initiative, and as part of a team

Desirable Criteria

  • Previous experience of working in a healthcare environment
  • Previous experience of using hospital-based systems
  • Previous experience of using specific systems relating to cancer care
  • Competent in using Microsoft Office, Excel, PowerPoint, and Word

Additional Criteria

  • Committed towards, and understanding of, the principles of equality, diversity, and inclusion
  • Genuine desire to work in an administrative environment
  • Genuine interest in improving cancer care and support for patients

Applicant Requirements

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
