Job Summary
The Patient Follow-Up Coordinator is responsible for the day-to-day administration and coordination of Remote Monitoring and Supported Self-Management Pathways (SSMP) within the Cancer Support Team. The post holder will manage patient activity across remote monitoring systems, ensuring accurate data entry, monitoring pathway compliance, undertaking quality assurance checks, identifying and escalating data gaps, and tracking patients throughout their follow-up journey.
Working closely with Cancer Support Workers, Cancer Nursing teams, Prehabilitation services and other multidisciplinary colleagues, the post holder will support the delivery of safe, effective and patient-centred follow-up care for people living with and beyond cancer. The role requires strong organisational skills, attention to detail, and the ability to manage competing priorities while maintaining high standards of data quality and patient confidentiality. The post holder will also provide administrative and project support to the Cancer Support Team.

Qualifications
Essential
- Experience in an administrative or coordination role involving data entry, record management and managing competing priorities.
- Experience organising meetings, maintaining databases and producing accurate records.
- Experience of tracking cases, maintaining accurate records and monitoring deadlines or key milestones.
- Demonstrable experience using electronic systems and databases to input, update and retrieve information accurately. Strong attention to detail and ability to identify and escalate data discrepancies.
Desirable
- Previous NHS or healthcare administration experience.
- Previous experience working in Cancer/oncology
Knowledge
Essential
- Excellent verbal and written communication skills with the ability to communicate professionally with patients, carers, clinicians and external organisations.
- Ability to work effectively within a multidisciplinary team and build positive working relationships across departments and organisations.
- Understanding of confidentiality, information governance, data protection and the importance of accurate patient records. Commitment to maintaining high standards of data quality and patient safety.
- Strong customer service skills with a patient-centred approach. Demonstrates compassion, professionalism and responsiveness when supporting patients and carers.
- Ability to work proactively and flexibly, manage workloads effectively, work independently and as part of a team, contribute to continuous improvement, uphold equality, diversity and inclusion principles, and support the delivery of high-quality cancer services and patient experience.

Desirable
- Experience using NHS systems (e.g. Lorenzo, EPR, Somerset Cancer Registry).
- Experience of remote monitoring platforms.
- Previous experience working within cancer services.
