We have an exciting opportunity for a Patient Safety Manager to join the Health in Justice service at Practice Plus Group.
You will work across the northern directorate, which covers our prisons across the north west of England and the whole of Yorkshire.
This role is home based, with travel to sites across the directorate as required.
This is a senior clinical leadership and patient safety role. You will be responsible for leading the patient safety agenda across the directorate, ensuring compliance with national patient safety frameworks, supporting investigations, driving quality improvement, and promoting a culture of learning and continuous improvement.
You will have the opportunity to work closely with operational leaders, Heads of Healthcare, commissioners, NHS England, and other stakeholders across our prisons and Immigration Removal Centres (IRCs).
Salary & Hours
This position is full time working 37.5 hours per week.
You will receive an annual salary of £50,000 to £60,000 per annum.

- Provide visible leadership for patient safety across the directorate.
- Lead and support Patient Safety Managers within the directorate.
- Promote a culture where staff feel confident reporting incidents, identifying risks, and learning from mistakes.
- Act as the key link between the region and NHS England Quality Assurance Leads.
- Oversee incident trackers and action plans.
- Ensure timely and accurate reporting of patient safety incidents.
- Lead responses in line with the Patient Safety Incident Response Framework (PSIRF).
- Support and coach teams completing Patient Safety Incident Initial Reviews (PSIIRs).
- Lead Clinical Case Reviews and coordinate investigations when serious incidents occur.
- Provide quality assurance of investigation reports and ensure submissions meet NHS England requirements.
- Identify trends and themes in incidents.
- Lead thematic reviews and improvement programmes.
- Provide assurance regarding the quality and safety of services.
- Contribute expert patient safety advice to governance and safety panels.
- Coach and develop leaders and frontline teams.
- Promote clinical supervision and evidence-based practice.
- Support teams to maintain high standards of care and governance.

Qualifications, skills and experience required:
- A registered clinician with relevant professional registration, such as: Registered Nurse (NMC) or Paramedic (HCPC registered).
- Significant post-registration clinical experience.
- Experience in patient safety, governance, risk management, or senior clinical leadership.
- Experience in leading teams or influencing multidisciplinary teams to improve performance and quality.
- Experience of healthcare incident investigations.
- Experience in Health in Justice services and/or understanding of deaths in custody processes.
- Experience of working with NHS England investigation and governance processes.
- Applying the Patient Safety Incident Response Framework (PSIRF).
