About the Role
Warwickshire’s Separated Children and Leaving Care Team work with care experienced young people aged 16-25 and young people who are separated children in care and care experienced. This is one service, but the team is split regarding staff and workloads for separated Young People and the Young People Leaving Care, with a duty rota covering both services. The Separated Children and Leaving Care Team is a county-wide service with offices in the North and the South of the county to enable access for all young people.
We are seeking 2 passionate and dedicated people to join our Leaving Care Service. This is a rewarding opportunity for someone who believes that young people leaving care deserve the best possible start in adulthood and are able to adapt their work style to the individual needs of each young person.
Now is an exciting time to be part of our team as Warwickshire begins to embed our Families First teams who support the young people in our County. Our approach is about locality working, multi-disciplinary and multi-agency teams that support the whole family. Building strong partnerships with local services, Health, Police, and Education.

In this role, you will:
- Work with Young People aged 16-25
- Manage a caseload and respond to the needs of those Young People on a day-to-day basis
- Engage in duty for the team, supporting other young people who aren't allocated to you
- Work with the Young People to support and get them ready for independence
