Personal Advisor - Leaving Care Team – 3 Month Contract
Location: Shropshire
Rate: £24.59 per hour (Umbrella)
Contract: 3-Month Contract (with potential for extension)
Hours: Full Time
Working Hours:
- Monday to Thursday: 8:45 am – 5:00 pm
- Friday: 8:45 am – 4:00 pm
DBS: Enhanced DBS with Adults' & Children's Barred List Required
Driving Requirement: Full UK Driving Licence Required
Pay: Bi-Weekly
Summary
We are seeking a dedicated Personal Advisor to join our Leaving Care Team on a 3-month contract, with the possibility of extension. This role is crucial in guiding young people as they transition from care to independent living. You will handle a varied caseload, including care leavers, asylum-seeking youth, and those with disabilities or unresolved immigration issues. Collaborating with partner agencies, you will provide personalized support to foster positive outcomes and equip young people with the skills necessary for a successful transition to adulthood.
Key Responsibilities
- Oversee a diverse and complex caseload of care leavers.
- Collaborate with young people to set and achieve personal goals.
- Conduct needs assessments and create individualized support plans.
- Develop and review Pathway Plans in accordance with statutory guidelines.
- Ensure Pathway Plans are SMART, outcome-focused, and promote independence.
- Assist young people in their transition to adulthood.
- Coordinate care planning with partner agencies and professionals.
- Encourage positive outcomes in housing, education, employment, and health.
- Build strong, trusting relationships with care leavers and their networks.
- Empower care leavers to engage in decision-making and service development.
- Work independently with young people and their families when needed.
- Prepare clear, accurate reports and maintain case records.
- Utilize supervision and team discussions for effective decision-making.
- Uphold professional boundaries and pursue ongoing professional development.

Essential Requirements
- Experience working with Care Leavers, Looked After Children, or vulnerable youth.
- Proven ability to manage a diverse and complex caseload.
- Understanding of safeguarding and corporate parenting duties.
- Skilled in conducting assessments and creating person-centered support plans.
- Ability to collaborate effectively with various agencies and professionals.
- Experience fostering positive relationships with diverse young people.
- Strong communication, report-writing, and organizational abilities.
- Capability to work independently and manage workload efficiently.
- Full UK Driving Licence and access to a vehicle.

Desirable
- Experience within a Leaving Care Service.
- Familiarity with legislation and statutory guidance for Care Leavers.
- Experience supporting asylum-seeking youth and those with immigration challenges.
- Experience working with young people with disabilities and additional needs.
