Summary
This role involves providing high-quality support and guidance to young people transitioning from care to independent adulthood. As a Personal Advisor, you will play a crucial role in assisting care leavers by developing, implementing, and reviewing Pathway Plans. Your efforts will ensure that these young individuals have access to the necessary services, resources, and opportunities to achieve positive outcomes in education, employment, training, accommodation, health, and wellbeing. This position is a 3-month contract with a local authority, offering a rate of £25.03 per hour under an umbrella arrangement.
Responsibilities
- Serve as the primary contact and key professional for care leavers.
- Offer advice, guidance, advocacy, and practical support to young people transitioning from care to independence.
- Conduct assessments and contribute to the development, implementation, and review of Pathway Plans.
- Assist young people in achieving the goals outlined in their Pathway Plans.
- Coordinate support services and maintain effective partnerships with internal departments and external agencies.

Requirements
- Experience working with children, young people, or families within social care, education, youth services, or the voluntary sector.
- Proven track record in supporting vulnerable young people to achieve positive outcomes.
- Experience in a multi-agency environment.
- Proficiency in developing and reviewing support plans, care plans, or pathway plans.
- NVQ Level 3, Diploma Level 3 in Children and Young People's Services, Youth Work, Social Care, or an equivalent relevant qualification.

Additional Information
- Working hours are 37 hours per week, with at least two days required in the office.
- The place of work is located at 858 Chester Road, Chelmsley Wood, Solihull, B37 7WG.
- Office hours are Monday to Thursday from 8.45 am to 5.20 pm, and Friday from 8.45 am to 4.30 pm.
- The role requires travel, necessitating business insurance, though a DVLA check is not required.
- An enhanced DBS with a barred list is required.
- Payments are made on a bi-weekly schedule.
