Are you a dynamic and motivated pharmacist looking to make a real impact in urgent and emergency care? Join our innovative team at Ysbyty Gwynedd working as part of the front door admissions team within the Emergency Department, Same Day Emergency Care, Medical Admissions and Surgical Admissions wards.
This is an exciting opportunity to work at the forefront of clinical pharmacy within a fast-paced, patient-focused environment. You will be embedded in the multidisciplinary team, supporting safe and effective medicines use from the point of entry into hospital care through to supporting the transition of patient care back into the community.
We are committed to providing the best learning and supportive experience for pharmacists and believe this is an ideal stepping stone towards specialist/advanced practice. This role will provide opportunity to build on your current prescribing skills and you will be supported to enrol on the advanced clinical practice pathway and gain core advanced level credentialing with the Royal College of Pharmacy. You will have mentorship from experienced advanced and consultant pharmacists.
Our clinical pharmacy team provides a high quality, patient-centred service ensuring safe and effective use of medicines across the integrated health community. As a pharmacist working in our team, you will deliver excellent pharmaceutical care and medicines optimisation to achieve the best outcome for our patients.
Main Duties Of The Job
Your responsibilities will include:
- Providing expert clinical pharmacy input to patients in our front door and admissions areas.
- Utilising your independent prescribing qualification to support medicines reconciliation, optimisation, and discharge planning.
- Providing wider clinical pharmacy services within the hospital.
- Collaborating closely with medical, nursing, and allied health professionals to ensure seamless patient care.
- Contributing to service development and quality improvement initiatives.
- Supporting the development of current and future pharmacy professionals, as well as other healthcare professionals, through clinical mentorship.

Qualifications And/or Knowledge
Essential criteria
- MPharm degree or equivalent
- Registered with the General Pharmaceutical Council
- Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications
Desirable criteria
- Accredited or eligible to be accredited as a HEI Clinical Pharmacy diploma/MSc Tutor
- Non-medical prescribing qualification
- RPS Member
Experience
Essential criteria
- Broad post registration experience
Desirable criteria
- Specialist in specialist clinical area

- Good verbal/written communication skills
