Role Overview
West London NHS Trust has an exciting opportunity for a motivated and patient-focused Pharmacy Technician to join the Integrated Care Pharmacy Team based at Clayponds Hospital. This varied role offers the chance to work across community hospital wards, community health services and clinics, supporting the delivery of high-quality medicines management and medicines optimisation services.
Responsibilities
- Work closely with pharmacists, nurses and the wider multidisciplinary team to ensure patients receive safe, effective and timely access to their medicines.
- Provide patient-centred medicines management and medicines optimisation services across Clayponds Hospital and community health settings.
- Support medicines reconciliation, patient counselling, medicines supply and discharge planning, helping to ensure safe and effective use of medicines.
- Undertake medicines management visits to wards and community services.
- Support stock management activities.
- Participate in audits and quality improvement projects.
- Contribute to the delivery of a responsive pharmacy service.

Requirements
- GPhC-registered Pharmacy Technician.
- Excellent organisational and communication skills.
- Ability to work effectively both independently and as part of a team.
- Desirable: Accredited Checking Technician status and medicines management qualifications.
