PHO Operations Co-ordinator - Community Care & Arataki PHO

Be the connection between primary care practices and the PHO — supporting practices, coordinating programmes and helping improve health outcomes for our communities.

We are looking for an organised, relationship-focused and proactive PHO Operations Co-ordinator to join our Community Care PHO team.

This is a key role supporting the day-to-day relationship between the PHO and our general practice network. You will work closely with practice owners, managers, GPs, nurses and wider practice teams to ensure PHO programmes, services and operational processes are well communicated, effectively implemented and supported across the network.

About The Role

Reporting to the Head of Community Care PHO, you will be a key point of contact for practices across the network, providing practical advice, operational support and coordination across PHO services and programmes.

You will build strong relationships with practices, identify opportunities for improvement, support quality initiatives and help ensure practices have the information and support they need to successfully deliver PHO programmes.

The role combines relationship management, programme coordination, communication, quality improvement, administration and event coordination, making it ideal for someone who is organised, solutions-focused and confident working across a diverse stakeholder group.

What You'll Be Responsible For

Practice relationships and engagement

  • Act as a key day-to-day contact for general practices on PHO services, claiming, systems and processes.
  • Build and maintain positive, trusted relationships with practice owners, managers, GPs, nurses and wider practice teams.
  • Understand practice needs and identify opportunities to improve services and processes.
  • Provide advice and support on PHO-funded programmes and initiatives.
  • Facilitate communication between practices and internal PHO teams.
  • Coordinate and resolve operational issues, escalating matters where appropriate.

Communication and coordination

  • Prepare and coordinate regular written and verbal communications to practices.
  • Support the implementation and ongoing management of the provider portal.
  • Contribute to clinical newsletters and key PHO communications.
  • Coordinate PHO Board and Clinical Advisory Group meetings, webinars and related activities.
  • Ensure practices receive clear, timely and consistent information about programmes, services and operational changes.

Programme and operational support

  • Coordinate the implementation of PHO programmes and projects across practices.
  • Monitor programme delivery and support practices to achieve agreed targets and milestones.
  • Ensure operational processes are documented, communicated and consistently applied.
  • Support the planning and delivery of practice forums, training, workshops and meetings.
  • Assist with service improvement initiatives.
  • Coordinate responses to operational issues affecting practices, working closely with Regional Business Managers and relevant PHO teams.

Quality improvement and data

  • Support practices to develop quality annual plans and monitor progress against quality improvement objectives.
  • Help ensure quality improvement activity aligns with flexible funding requirements.
  • Analyse performance and service utilisation data to identify trends, opportunities and areas for improvement.
  • Promote best practice and continuous quality improvement approaches.
  • Provide PHO oversight and coordination of NZPES/IPSOS updates and reporting, sharing key trends and learnings across the network.

Health equity and population health

  • Champion equity-focused approaches across PHO services and programmes.
  • Support practices to identify and address barriers to healthcare access.
  • Contribute to initiatives that improve outcomes for Māori, Pacific peoples, rural communities and other priority populations.
  • Promote culturally safe, responsive and accessible service delivery.

About You

You will be a confident and highly organised professional who enjoys building relationships, coordinating activity and supporting people to achieve shared outcomes.

You will ideally have:

  • A relevant tertiary qualification and/or demonstrated experience in health management, public health, nursing, health sciences, business management or project management.
  • Experience working in primary health care, PHO, NGO, community health or public health settings.
  • Proven experience coordinating operational activities and projects.
  • Strong written and verbal communication skills.
  • Experience building and managing relationships with a range of stakeholders.
  • Confidence interpreting and using health data for reporting and decision-making.
  • Excellent organisation skills and the ability to manage multiple priorities and deadlines.
  • A practical, solutions-focused approach and the ability to follow issues through to resolution.

Position Details

This is a Permanent Full Time position working 40 hours per week across 5 days - Based in our head office in Parnell.
