About the Role

Location: Westborough, MA (with community-based work)
We are seeking a licensed, compassionate Physical Therapist to support individuals enrolled in the ABI/MFP Waiver Programs as they work toward greater independence and quality of life. Under the supervision of the Physical Therapy Program Manager, the PT provides individualized, goal-driven therapy in a variety of settings—including participants’ homes, our Training & Rehabilitation Center (TRC), and the community. This role is ideal for a clinician who values functional, real-world therapy and thrives in a collaborative, interdisciplinary environment.
What You’ll Do
- Provide physical therapy services focused on mobility, safety, functional independence, and overall quality of life
- Develop and implement individualized treatment plans aligned with participant goals, needs, and interests
- Collaborate closely with participants, families, caregivers, service coordinators, and the interdisciplinary team
- Select and adapt therapeutic activities that support physical and cognitive abilities
- Train and educate direct care staff to reinforce therapeutic goals and promote carryover
- Recommend adaptive equipment and environmental modifications; write letters of medical necessity as needed
- Provide protocols and training for new or updated equipment in residential settings
- Complete timely documentation and billing to meet reimbursement requirements
- Participate in meetings, trainings, and weekly supervision
- Mentor PTA’s, students, and interns as assigned

Qualifications
- Licensed Physical Therapist (required)
- Minimum of 2 years preferred experience, ideally with:
- Acquired Brain Injury (ABI) population
- Outpatient, home care, or community-based therapy
- Strong communication, collaboration, and organizational skills
- Commitment to individualized, person-centered care
- Active license in good standing and participation in continuing education
