Part time - 22.5 hours
Are you a Band 5 Physiotherapist looking to develop your skills at a Band 6 level? Are you a Band 6 Physiotherapist who would like to develop your clinical skills in a different area?
If so, we have a brilliant opportunity to come and join us here in the District Therapy Team at Bolton Foundation Trust. We work across six neighbourhoods in Bolton to help provide a holistic community-centred provision.
We are seeking to appoint a dynamic, confident, innovative, and flexible individual with excellent communication and organizational skills. The successful individual will need to engage closely with acute staff, community colleagues, and social care staff to coordinate and provide high-quality care in the community to avoid hospital admission and support hospital discharges.

Service is over 7 days, 8 am - 6 pm service. Staff currently work 1 in 6 weekends.
In order to successfully undertake this post you must be an experienced Physiotherapist looking to develop in this clinical area.
You will need some knowledge of community/intermediate care services.
You will be an effective team player, inspirational, flexible, adaptable, enthusiastic, and fully committed to the development of patient-focused, clinically effective, and efficient services.
The role will be part of a multi-disciplinary team consisting of Physiotherapists, Occupational Therapists, Therapy Instructors, and Assistant Practitioners.
The service is part of the Neighbourhood model and has 6 bases across Bolton.

You will be a key player in providing assessment, personalized care planning, and management of care to patients who may require intensive home support as an alternative to hospital care. The role will include participating in effective multi-disciplinary team meetings and integrated working with the Local Authority.
