Are you an experienced and motivated Physiotherapist looking to take the next step in your career?
We are offering an exciting 12-month fixed term/secondment opportunity within the Coventry Integrated Neighbourhood Teams – a pioneering service that brings together health and social care professionals to deliver seamless, person-centred care.
About the Role
As a Physiotherapist, you will manage a varied caseload, providing specialist assessments and evidence-based interventions to patients with a wide range of health conditions across community settings, including patients' homes and short-term placements.
- You will be part of an innovative team supporting reablement, hospital discharge, and admission avoidance across urgent and emergency care pathways.
- Working collaboratively with multidisciplinary colleagues, you will help shape and influence the delivery of integrated care for the people of Coventry.
Key Responsibilities
- Using your clinical expertise and sound professional judgement, you will work collaboratively with colleagues, patients, carers, social care partners, and third-sector organisations to support individuals to achieve their goals, maximise independence, and remain at home wherever possible.
- You will undertake holistic assessments, develop tailored care and therapy plans, and deliver interventions that address patients' physical, cognitive, psychosocial, behavioural, sensory, and environmental needs.

Requirements
- The successful candidate will possess strong knowledge across a variety of clinical areas, including neurological, orthopaedic, musculoskeletal, respiratory, falls, and care of older people.
- You will also contribute to service improvement initiatives, support effective information management and data quality, and work alongside team leaders and managers to enhance the delivery of integrated community services across Coventry.
