Working as part of the multidisciplinary team, you will assess, plan, implement and evaluate patient care programmes, using evidence-based practice to meet the rehabilitation needs of patients. You will adopt a holistic, patient-centred approach, ensuring patients and carers are actively involved in decision-making and goal setting.

You will contribute to effective discharge planning and work collaboratively with colleagues across health and social care to facilitate safe and timely discharges, supporting the reduction of unnecessary inpatient length of stay where appropriate.

You will also provide physiotherapy advice and support to ward staff, patients, carers and partner organisations, promoting effective communication and continuity of care to achieve the best possible outcomes for patients.

The Ideal Candidate

We are looking for someone who:

• Holds the necessary professional physiotherapist qualification and is registered with the HCPC.
• Has experience working in a relevant Healthcare setting.
• Demonstrates a commitment to high-quality service delivery and continuous improvement.
• Possesses strong communication and interpersonal skills to build positive relationships with service users and colleagues.
• Is able to work effectively within a multi-disciplinary team to deliver integrated solutions.
• A valid UK driving licence and access to a car are essential for this role.

Desirable:

• Additional qualifications or training relevant to the role.
• Experience working in a multi-disciplinary environment.
• Knowledge of industry best practices and innovative service delivery models.
• A desire to further professional development through additional training or qualifications.
