Exciting Opportunity for Band 6 Physiotherapist
An exciting opportunity has arisen in a community rehabilitation team for a band 6 physiotherapist to join a supportive and enthusiastic community therapy service as a Physiotherapist, working across community delivering rehabilitation in patients own homes following discharge from an acute bed, or supporting Long term condition management.
About the Team
The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.
Responsibilities
- Working with patients with a large variety of conditions and disabilities, in their own homes and local community.
- Responsible for assessment, intervention, and discharge of a caseload of patients.
- Act as the direct supervisor to staff.
- Provide support and expertise to the other members of the team.

Training and Support
We offer a fabulous training programme within a supportive team.
Working Hours and Location
The service operates across 5 days, Monday to Friday. Your nominated base will be Blackburn with Darwen, however cross locality working across the East Lancs footprint at times.
Role Description
The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment of patients referred to the service. Responsible for managing a caseload of patients with a variety of complex conditions including long term condition management and acute hospital discharges. You will see patients in own homes, working with an MDT in other organisations. You will carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention.
