An Exciting Opportunity for an Experienced HCSW
An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS).
About the Role
The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users in their own home or as out-patients.
The primary function of this role will be to work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. Therapists will provide specialist assessments to this patient group and allocate work to this post holder. The post holder will also be expected to hold their own caseload, assess, progress and discharge autonomously within parameters.
Key Responsibilities
- Work with the Physiotherapy Service within Intermediate Care.
- Deliver therapeutic treatment programmes to service users in their own homes.
- Prevent hospital admission and support early discharge from hospital.
- Provide specialist assessments to this patient group.
- Hold their own caseload, assess, progress, and discharge autonomously within parameters.

- HNC in Health and Social Care or equivalent or SCQF level 8 e.g. Postural Stability Instructor or equivalent.
- Experience working in the NHS as a therapy support worker.
- Experience working with older people as they are the main group of service users referred.
Additional Information
- Working as part of a supportive multidisciplinary team.
- Excellent organisational skills.
- Excellent communication skills.
- Ability to work independently as well as within the team.
- There may be occasions where you will be required to work in one of the other community hospital sites.
- Public Holiday working is in operation.
- Regular supervision, PDP and supports training opportunities.
- Ability to travel throughout the area is essential for this community post.
