Sounds great, what will I be doing?
In this role, you will manage a caseload of service users with complex health and support needs, helping them maintain their wellbeing, independence, and achieve personal goals. You will provide day-to-day care and support in line with individual care plans, assist with educational, leisure, and health-related activities, and contribute to the planning and review of support plans.
Responsibilities include:
- Supporting medication administration
- Monitoring health, safety, and wellbeing
- Reporting concerns promptly
- Carrying out domestic tasks where required
- Liaising with partner agencies and carers
- Supporting high-quality housing management services in compliance with organisational policies and procedures

You will need:
- A strong understanding of mental health, including the signs, symptoms, and treatments of common conditions, and the ability to recognise deteriorating mental health and take appropriate action
- Commitment to promoting independence by motivating service users to achieve their goals and participate fully in daily activities
- Strong literacy, numeracy, and IT skills, including experience using Microsoft Word, Outlook, and electronic support planning systems
- Knowledge of safeguarding, health and safety, and equality and diversity principles
- Willingness to provide personal care, support domestic tasks, travel between services, and work flexible shifts, including evenings, weekends, and sleep-ins
