Role Description
The Practice Nurse will provide direct patient care and clinical support within Woden Road Surgery’s primary care setting. Day-to-day responsibilities include:
- Conducting patient assessments
- Managing long-term conditions (such as diabetes, asthma, and cardiovascular disease)
- Administering vaccinations and injections
- Performing wound care
- Supporting health promotion and disease prevention initiatives

The role also involves maintaining accurate clinical records, adhering to relevant clinical guidelines and policies, and working closely with general practitioners and other team members to coordinate patient care. This is a part-time, on-site role based in Wolverhampton, requiring regular presence at the surgery to support face-to-face patient services.

Qualifications
- Registered Nurse (RN) qualification with current registration (or eligibility for registration) with the appropriate nursing regulatory body.
- Experience in primary care, community nursing, or a related clinical setting, including chronic disease management and preventive care.
- Strong clinical skills in assessment, wound care, vaccination administration, and basic diagnostic procedures (e.g., blood pressure monitoring, blood tests within scope of practice).
- Ability to maintain accurate and timely clinical documentation, and to use electronic health record systems or practice management software.
- Effective communication and interpersonal skills for working with patients, families, and multidisciplinary team members, including the ability to discuss sensitive health issues with empathy and clarity.
- Commitment to patient-centered care, safeguarding, infection prevention and control, and adherence to clinical governance standards.
- Organizational and time-management skills to balance scheduled clinics, urgent patient needs, and administrative tasks in a busy practice environment.
- Willingness to participate in continuing professional development, training, and practice meetings to keep knowledge and skills up to date.
- Experience or interest in health education and promotion, including lifestyle advice on topics such as smoking cessation, weight management, and cardiovascular risk reduction.
