Practice Nurse

Be part of the nursing team that will change how primary care is delivered in your Brighton community.

This is an opportunity to join our Brighton primary care team at an important point in its journey.

Over the last two years, we've been transforming our practices, strengthening the way we work and creating a clearer vision for the future. We're now bringing our Sussex practices much closer together, sharing expertise and support across our services and creating stronger teams.

We're now taking the next step.

Allied Medical Practice and Ship Street Surgery are coming together as one group, with plans to move into purpose-built practices next year. This will give us the capacity to significantly grow our patient population and create a modern primary care service for the future.

That's where you come in.

We're looking for a Practice Nurse who wants to be part of building that future.

This isn't simply about stepping into an established team and carrying on as before. You'll be joining a service that is evolving, with the opportunity to contribute ideas, improve how we work and help create a positive, high-performing clinical culture.

We're ambitious about what these practices can become - and we're looking for people who are ambitious about the care they provide.

What You'll Focus On

  • Delivering excellent patient care

    You'll provide high-quality, evidence-based nursing care across a broad range of primary care services, working both independently and as part of a multidisciplinary team.

    You'll assess, treat, monitor and support patients while building trusted relationships and ensuring care is safe, compassionate and centred around the individual.

  • Managing long-term conditions

    You'll support patients living with chronic conditions, undertaking assessment, monitoring and management planning to help improve outcomes and support people to manage their health.

    You'll contribute to QOF and Enhanced Services activity, ensuring appropriate investigations are completed and accurately recorded.

  • Health promotion and prevention

    You'll play an important role in helping patients stay well, identifying health risks and supporting people to make positive changes.

    You'll contribute to health checks, smoking cessation, preventative care and wider health promotion initiatives.

  • Wound care and clinical procedures

    You'll assess and manage routine wounds and trauma and support more complex wound management where appropriate.

    You'll also have the opportunity to use and develop your skills across a broad range of primary care procedures, including; Immunisations and vaccinations, Travel health, Ear care, Phlebotomy, ECGs, Blood pressure monitoring, Blood glucose monitoring, Spirometry, Minor procedures, Therapeutic monitoring etc.

  • Screening and health checks

    You'll contribute to screening programmes and health assessments across areas including women's and men's health, cardiovascular health and sexual health.

    You'll identify concerns, provide appropriate advice and work with patients to develop plans that support better health outcomes.

  • Working as part of a strong clinical team

    You'll work closely with GPs, Practice Nurses, Healthcare Assistants, General Practice Assistants and the wider multidisciplinary team.

    We're building a culture where people support each other, share knowledge and take collective responsibility for delivering great patient care.

    You'll also be part of the wider IC24 primary care community across Sussex, giving you access to colleagues, expertise and support from other practices.

Travel to other IC24 sites may occasionally be required to support service needs.
