Practice Nurse 18 Hours per week
We are looking for a caring and enthusiastic Practice Nurse to join our friendly, supportive and patient-centred team for 18 hours per week due to our expanding patient population. The working days will be agreed with the successful candidate.

Our multi-disciplinary approach enables us to deliver high quality, patient-centred care to our Patients and Community.
Key Responsibilities For The Position Are
- Managing Long-term Conditions including Diabetes, Asthma, COPD, Hypertension, CKD and CHD
- Keeping up to date with Chronic Disease Practices and Guidelines.
- Reviewing Chronic Disease Test Results (including bloods)
- Delivery of Lifestyle Advice and Health Promotion
- Ideally a Non-Medical Prescriber
- Attend and participate in Practice Meetings
