Viaduct Care CIC is the GP Federation for Stockport, operating as a not-for-profit organisation owned by 32 local GP practices. The federation focuses on developing innovative solutions to the challenges faced by the NHS, while supporting and strengthening general practice. Viaduct Care aims to ensure that patients continue to receive high-quality, accessible care across the community. Team members work collaboratively with practices and partners to improve services and outcomes for the local population.
Role Description
The Practice Support Pharmacist is a full-time hybrid role, based across Stockport. This role involves providing medicines optimisation support to GP practices, reviewing prescribing, and helping to implement safe, evidence-based treatment plans. Daily tasks include:
- Conducting medication reviews
- Supporting long-term condition management
- Responding to medicine-related queries from clinicians and patients
- Contributing to multidisciplinary meetings

The post holder will help develop and deliver practice-level initiatives that improve prescribing quality, reduce medication-related risk, and support adherence to local and national guidelines. The role also includes:

- Maintaining accurate clinical records
- Participating in audits and service evaluations
- Contributing to continuous service improvement across the federation
Qualifications
- Registration as a Pharmacist with the General Pharmaceutical Council and relevant post-qualification clinical experience in primary care or community settings.
- Strong clinical knowledge of medicines management, long-term condition therapies, and current NHS prescribing guidance.
- Skills in medication review, medicines reconciliation, and safe prescribing support, including awareness of polypharmacy and high-risk drugs.
- Ability to interpret clinical guidelines and audit data, and to translate findings into practical changes in practice.
- Clear, professional communication skills for working with GPs, practice teams, and patients, including the ability to explain complex information in accessible terms.
- Competence in using electronic health records, prescribing systems, and standard office software to document care and produce reports.
- Strong organisational and time-management abilities to prioritise workload across multiple practices and work effectively in a hybrid model.
- Commitment to person-centered care, equality, and inclusion, with a collaborative approach to multidisciplinary working.
- Evidence of ongoing professional development; independent prescribing qualification or work toward it is an advantage.
- Experience in quality improvement, service development, or participation in practice-based audits is desirable.
