To be an active member of the multidisciplinary team by using professional judgement and assuming responsibility and accountability for assessing, implementing and evaluating clinical care for patients to meet their needs in a safe caring environment that promotes dignity, privacy and respect.

Patient / Client Care

  • Of Nurse Led Care To a Defined Caseload Supporting Patients Along Clinical Pathways And Protocols Of Care.

  • Deliver Nurse Led Pre-Assessment Service, Developing Advanced Professional/clinical Skills/knowledge In Areas Such As; Detailed Clinical Assessment Of Patients, Consent, Ordering And Interpretation Of Pathological Tests.

  • Supports Patients To Be Partners In Their Own Health Care Through Education, Discussion And Explanation Of Care.

  • Works As a Member Of The Clinical Team.

  • Work In Partnership With Others To:

    • Ensure highest standards of patient care and safety.
    • Coordinate the care for patients in specialised area of practice.
    • Contribute to the delivery of a planned specialised programme of nursing care.
    • Autonomy as an independent Pre-Assessment Nurse.
    • Initiate assessment, request of relevant investigations. Initiate appropriate treatment plans and liaising with other primary and secondary teams to facilitate concordance with treatment plans.
    • Involvement of comprehensive patient management and appropriate documentation and liaison with surgical team.
    • Act as a resource providing advice, support to patients, carers and the multi-disciplinary team.
    • Perform clinical examination, assessment and undertakes a comprehensive medical history.
    • Order investigations (ECHO, bloods, ECG, Swabs and other specialist tests).
    • Check results and report abnormalities, liaise with medical team when appropriate.
    • Competent to co-ordinate and manage own workload.
