Job Summary
As a member of Haringey GP Federation Primary Care Clinical Team, the post holder will provide high quality clinical care and operational support to the Haringey Serious Mental Illness (SMI) Physical Health Check Service and GP @ The Grove and HAGA Service (substance misuse). You will be working with several providers across several sectors: primary care, community health, hospital, social care and the voluntary sector.
Your primary focus will be conducting SMI Physical Health Checks across Haringey GP surgeries, where your work will directly help optimise the health of vulnerable patients, reducing their risk of developing serious, life-threatening conditions such as heart attacks and strokes.
Additionally, you will play a vital role in supporting a specialised GP clinic hosted within HAGA (Haringey Alcohol Service) once every two weeks, providing essential healthcare continuity to patients overcoming alcohol addiction.
Main duties of the job
The Aims Of This Post Are To:
- Increase the number of physical health checks performed on people with SMI and substance misuse who may be at increased risk of COVID-19 and its subsequent complications.
- Increase the overall number of physical health checks performed on people with SMI in Haringey from 21% in 2018 to the national target of 75% as of 2026.
- Encourage a more holistic approach to healthcare for patients through collaborative working with North London Foundation Trust and the Haringey Wellbeing Network at Mind in Haringey.
The Post Holder Will Work Closely With The SMI Community Mental Health Allied Professional, HAGA Services And Service Lead To Deliver On Key National And Local Strategies To Reduce Unwarranted Variation In The Physical Health Outcomes Of Patients With SMI And Substance Misuse By:
- Increasing the proactive identification, monitoring and management of physical illness in this patient cohort.
- Rapidly identifying at-risk patients on the SMI register, impacted by or with potential higher risks to healthcare complications as a result of COVID-19.
- Improving engagement of SMI patients with primary and community care, including referral or signposting to appropriate services.
- Supporting practices maintain accurate SMI registers.
- Identifying and coordinating patient care with the Haringey Wellbeing Network.

Job Description
Job responsibilities
- The post holder will work in with the clinical lead and service manager to develop and operationalise the physical checks for the SMI pathway.
- The post holder will also work with the GP based at Haringey Advisory Group on Alcohol (HAGA) and service manager to perform comprehensive health checks for patients with substance misuse.
- This is a patient-facing role which will support improving the overall health for patients.

Essential Skills / Experience
- Excellent interpersonal skills, including observation, listening and empathy.
- Ability to communicate clearly both verbally and in writing sensitive information.
- Ability to provide and maintain a high quality of care.
- Ability to maintain accurate records.
- Ability to prioritise own workload in the face of competing pressures.
- Ability to work effectively with a range of stakeholders including people with SMI or Substance Misuse, voluntary groups, social work, acute providers and commissioners.
Desirable Skills / Experience
- Ability to interpret & implement local, national, Haringey GP Federation and Trust policies.
- Excellent presentation skills, both written and verbal.
- Experience of working in General practice.
- Experience of working with the IT clinical system EMIS.
- Experience of working in a multi-disciplinary setting.

Essential Additional Requirements
- Fitness Analysis as appropriate.
- Ability to travel across Haringey Practices efficiently to deliver the service and or attend meetings etc
Desirable Additional Requirements
- Current driving licence.
Knowledge
Essential
- A basic understanding and knowledge of the Mental Health Act and policies that will be adhered to in order to provide the appropriate level of care for this cohort.
- Understanding and knowledge of physical health promotion.
- Understanding of how a clinical health check is completed and the clinical tests that may be included.
- Clinical knowledge of how to carry out basic tests i.e. blood pressure, pulse, testing urine.
- Understanding of Infection Control and requirement to use PPE when necessary.
Essential Qualifications
- Good standard of general education including Maths and English at Grade C and above (or equivalent).
- Health and Social Care qualification such as Care Certificate or NVQ level 2 or 3.
- Good understanding of spoken and written English.
- Desire to develop skills and qualifications under continuous professional development.
- Trained in Phlebotomy.
- Experience in a patient facing role.
- Experience of working to achieve standard within the Quality and Outcome framework (QoF).
- Experience with chronic disease management.
Desirable Qualifications
- Knowledgeable use of the clinical system EMIS.
- General practice experience.
- Experience in managing those with SMI.
- Experience in managing those with Substance Misuse.
