Are you an ambitious, motivated nurse ready to take the next step in your leadership journey? HMP/YOI Feltham B is offering an exciting opportunity to join our team as Primary Care Service Manager and play a key role in shaping the future of integrated physical healthcare.

This is more than a clinical leadership role. It is an opportunity to lead change, develop people, improve patient care and influence the delivery of physical healthcare within a unique and complex environment.

Working closely with the Head of Healthcare and the wider multidisciplinary team, you will provide visible and inspiring clinical leadership to the physical health nursing team. You will help ensure the delivery of safe, effective and responsive care across primary and urgent care, long-term condition management, health screening, vaccination and health promotion. You will have an important role in driving clinical excellence, governance and continuous improvement, using data, audit, patient feedback and learning to identify opportunities and deliver meaningful change. You will also contribute to the development and implementation of the NMOC Integrated HCC, helping to ensure our physical healthcare services continue to develop around the needs of our population.

Main duties of the job

Clinical Leadership

  • Provide professional and clinical leadership for the Physical Health nursing team.
  • Ensure safe, effective, evidence-based nursing practice is delivered across all healthcare areas.
  • Act as a senior clinical decision-maker for complex physical health cases.
  • Support the management of patients with long-term conditions, complex health needs and deteriorating health presentations.
  • Promote high standards of patient-centred care and clinical excellence.
  • Provide visible clinical leadership during emergencies and operational incidents.

Service Delivery & Operational Management

  • Lead the day-to-day delivery of primary care and physical health services across the prison.
  • Ensure effective allocation of nursing resources and clinic capacity.
  • Oversee urgent care, medication administration, reception screening and scheduled clinics.
  • Monitor waiting times, appointment utilisation and patient access to care.
  • Support business continuity planning and service resilience during staffing pressures or operational disruptions.

Clinical Governance & Patient Safety

  • Drive continuous improvement in clinical quality, patient safety, workforce development, health promotion and service delivery.
  • Promote a culture of compassion, professionalism, accountability and excellence across the Physical Health nursing service.

Workforce Leadership & Development

  • Provide senior clinical and operational leadership for physical healthcare services across HMP/YOI Feltham B.
  • Ensure the delivery of safe, effective, responsive and high-quality physical healthcare to a complex prison population.

Public Health & Health Promotion

  • Contribute to the development and implementation of the NMOC Integrated HCC.

Education / Qualifications

Essential criteria

  • Registered General Nurse
  • Evidence of Continuous Professional Development
  • Degree Level Qualification

Desirable criteria

  • Non-Medical Prescriber
  • Management Qualification

Experience

Essential criteria

  • Substantial post registration experience in Primary care or an acute setting (Adult)
  • Experience of staff management, development, supervision and appraisal
  • Experience of joint working with other agencies

Desirable criteria

  • Prior experience of working in a secure or immigration setting
  • Experience of developing policies and procedures

Knowledge

Essential criteria

  • Knowledge of chronic disease and long term condition management
  • Knowledge of the principles of Best Value/Clinical Governance
  • Knowledge of current developments and practice

Desirable criteria

  • Knowledge of the principles of Quality management.
  • Knowledge of and participation in research.
