Job Summary

As Program Manager III for Community Health, you will manage the implementation and ongoing maintenance of designated programs while planning and developing systems and procedures to ensure efficiency and effectiveness. In this role, you will serve as a liaison between the department and external communities, provide coaching and mentorship to staff, and actively participate in the budgetary process to support program goals. The position is heavily strategy- and data-driven, focusing on measurable community health impact rather than managing community events, with the opportunity to manage programs that directly impact hospital compliance and broader community health.

A caregiver in this role works Monday through Friday from 8:00 a.m. – 5:00 p.m.

This position is onsite, with 60% of the time at Avon and 40% at Fairview.

A caregiver who excels in this role will:

  • Develop, implement, manage, and maintain operational systems and activities.
  • Serve as a liaison with all levels of the organization and outside the community.
  • Manage multiple priorities and projects with competing deadlines.
  • Allocate time to meet completion requirements.
  • Serve as a coach and mentor for other positions in the department.
  • Review and monitor adherence to Human Resources policies and corporate compliance procedures.
  • Manage all community health-related activities, specifically as they relate to the community health needs assessment.
  • Develop and lead programmatic strategies to address the priorities of assessment and hospital implementation strategies.
  • Responsible for developing, launching, and implementing hospital strategies required by the IRS.
  • Lead efforts in tracking progress and outcomes of strategic initiatives, setting short-term, mid-term, and long-term metrics for each project.
  • Launch new strategies and ensure they are executed according to plan, with measurable outcomes.
  • Prepare and report progress and outcomes of implementation strategies for compliance purposes.
  • Other duties as assigned.

Minimum Qualifications

  • Bachelor's Degree in Business Administration, Healthcare Administration or related field and five years of related experience
  • OR Master’s Degree and four years of experience
  • Knowledge of statistical and financial analysis, primary research and business plan development
  • Strong presentation, communication and training skills

Preferred Qualifications

  • Master’s degree
  • Healthcare settings
  • Experience and/or education in Public Health

Physical Requirements

  • Ability to communicate and exchange accurate information.
  • Ability to perform work in a stationary position for extended periods.
  • Ability to work with physical records or operate a computer or other office equipment.
  • In some locations, ability to travel throughout the hospital system.
  • In some locations ability to move up to 25 lbs.
