Programme Director of Continuing Health Care and Funded Nursing Care
The Programme Director of Continuing Health Care and Funded Nursing Care is responsible for leading the strategic transformation agenda across the Care Group for Primary Care and Community to deliver high-quality, integrated services that meet the needs of patients and the community.
Responsibilities
The Post Holder Will Have Delegated Senior Lead Reporting To The Executive Director Of Nursing For Continuing NHS Healthcare (CHC) Across The Health Board, Ensuring
- Robust governance and oversight
- Full compliance with the National Framework for CHC
- High-quality, person-centred care
- Effective financial stewardship and value-based commissioning
- Clear and consistent reporting and assurance to the Board
The role will act as the senior operational and strategic lead for CHC on behalf of the Executive Director of Nursing, leading system-wide improvement in partnership with Local Authorities and Regional Partnership Boards (RPBs), ensuring alignment with Welsh Government expectations for urgent improvement and national consistency.
The post holder will ensure the Health Board can provide clear assurance on CHC governance, quality, performance and financial risk, supported by robust data, audit and continuous improvement processes.
- Visionary leadership for the Care Group transformation agenda, ensuring alignment with national healthcare policies, including the CHC National Framework and National Commissioning Framework
- Lead the development and delivery of a Health Board wide CHC strategy and improvement plan aligned with National priorities and the emerging national CHC improvement programme
- Ensure all CHC governance risk and performance information is synthesised into clear assurance reporting to the Executive Director of Nursing, including escalation of risks, compliance issues and financial pressures
- Work collaboratively with other Directors in the development and delivery of a robust Development Plan, identifying current and future service and transformational needs to deliver integrated care. This will require extensive and ongoing research into the approach taken in other areas
- Lead on the design, development, and delivery of complex transformation programmes, including clinical service redesign, digital transformation, workforce optimisation and CHC pathway redesign
- Responsible for providing strategic overview, leadership and transformational change to enable the effective integration of services. Lead and facilitate the resolution of strategic issues and interdependencies and provide flexible targeted delivery support to the organisation to deliver the Transformation & Improvement priorities.
