About the Role
The Band 5 Registered Nurse will support the delivery of the Healthy Child Programme (0–19 years) within the 0–19 Service, promoting positive health, wellbeing and developmental outcomes for children, young people and their families. The post holder will provide targeted support to families with additional needs, including those where safeguarding concerns are identified, helping to reduce health inequalities and improve access to services.
Working under the supervision of a Band 6 Health Visitor or School Nurse, the post holder will contribute to safeguarding, assessment, intervention and care planning in partnership with families and agencies such as Children’s Services, Family Hubs, Action for Children, education providers and other health professionals.
The role includes delivering high-quality, safe and effective care, supporting health promotion, early help and targeted interventions, and contributing to service quality standards, KPIs, clinical governance, audit and service improvement activities.
The post holder will maintain accurate records, participate in safeguarding supervision, identify and manage risks, and ensure children and families receive timely, coordinated and person-centred support. We have full time and part time vacancies available that can be discussed during interview.

- Assess health needs and contribute to the planning, delivery and evaluation of care for children, young people and families across home, clinic, school and community settings.
- Work alongside Health Visitors and School Nurses to support vulnerable children and families, including those subject to CIN, CP and CIC plans, contributing to safeguarding assessments, interventions and reviews.
- Monitor the health, wellbeing and development of children at risk, supporting multi-agency plans that promote safety and positive outcomes.
- Provide targeted interventions under SCPHN direction.
- Act as Lead Professional, Key Worker or health representative where appropriate, coordinating support and communication between agencies.
- Deliver evidence-based health promotion, parenting support and interventions that improve wellbeing and reduce health inequalities.
- Recognise, record and escalate safeguarding concerns in line with local and national procedures.
- Participate in clinical, safeguarding and restorative supervision
- Develop effective partnerships with families and professionals, sharing information appropriately in accordance with safeguarding and information governance requirements.
- Support students, apprentices and new staff as appropriate.
- Maintain professional competence through training, appraisal and CPD.
- Keep accurate records and manage resources safely, working in accordance with the NMC Code, safeguarding procedures and Trust policies.

This includes the monitoring and provision of targeted support to vulnerable children and those whose welfare and safety may be at risk.
