Rapid Response Care Assistant/Support Worker

Choices Homecare are seeking Rapid Response Care Assistants/Support Workers for our clients in locations across the Wakefield and Kirklees region.

Rate of Pay: £13.50 per hour

Contract Type: Block paid on shift rotation

Hours Of Work: 7am - 2pm, 16.00pm - 22.00pm

Days Of Work: Week 1: Monday, Friday, Saturday, Sunday - Week 2: Tuesday, Wednesday, Thursday

About The Role

A Rapid Response Care/Support Worker should demonstrate compassion, reliability, communication skills, flexibility, observation skills, teamwork, resilience, and the ability to work independently in a fast-paced environment.

Key Responsibilities

• Provide appropriate support to service users during their first call or initial assessment, ensuring their needs, preferences, and circumstances are clearly understood.
• Gather all relevant information and complete or obtain the necessary documentation and paperwork to support the development and implementation of an effective care plan.
• Carry out care calls as required during the initial period of service, providing continuity and reassurance until the service user is successfully established on their regular care round.
• Undertake appropriate risk assessments, identifying potential or immediate risks to service users, staff, and others.
• Provide personal care, including washing, dressing, toileting, and continence support.
• Support with medication, following prescribed instructions and organizational procedures.
• Assist with mobility and transfers, using appropriate equipment and safe moving-and-handling techniques.
• Help with meals, hydration, and basic household tasks where required.
• Provide emotional support and reassurance, particularly during periods of illness, crisis, or change.
• Monitor and report changes in a person’s health, wellbeing, or behaviour to the appropriate professional.
• Work closely with nurses, social workers, occupational therapists, GPs, and other health and social-care professionals.
• Promote independence, dignity, choice, and person-centred care.
• Maintain accurate, confidential records and report incidents or safeguarding concerns promptly.
• Follow health and safety, infection control, safeguarding, and confidentiality policies.
• Manage changing priorities and work effectively under pressure while maintaining a calm and professional approach.
• Support people to remain safely at home and, where possible, prevent unnecessary hospital admission or facilitate a safe discharge from hospital.
