Reablement Support Worker – 3 Month Contract

Location: Swindon
Rate: £13.47 per hour
Contract: 3 Month Contract
Hours: Flexible Shifts, 5 Days Over 7 Including Weekends & Bank Holidays
DBS Required
Pay: Bi-weekly

Summary

We are seeking compassionate and dedicated Reablement Support Workers to join our dynamic Adult Health & Social Care team. This role is pivotal in assisting residents within their homes, focusing on restoring their confidence, independence, and daily living skills after illness, hospital discharge, or life changes. You will deliver personal care, practical assistance, and emotional support, ensuring the promotion of independence and dignity at all times.

Responsibilities

  • Aid service users in regaining and maintaining independent living skills.
  • Deliver personal care and assist with daily activities.
  • Support mobility, including moving and handling, and the use of specialized equipment.
  • Administer medication in accordance with care plans and policies.
  • Monitor progress and report on outcomes and evolving needs.
  • Collaborate with families, carers, health professionals, and social care teams.
  • Assist with shopping, financial management, and attending appointments as needed.
  • Participate in therapy programs and rehabilitation activities.
  • Keep accurate records and complete reports after each visit.
  • Provide compassionate support for end-of-life care when necessary.
  • Appropriately report any safeguarding concerns, incidents, and risks.

Essential Requirements

  • Experience in supporting vulnerable adults, the elderly, or those with care needs.
  • Knowledge of person-centered care and fostering independence.
  • Strong communication and interpersonal abilities.
  • Flexibility to work across various shifts, including evenings, weekends, and bank holidays.
  • Proficient literacy and numeracy skills.
  • Capability to maintain precise records and documentation.
  • Physical ability to perform moving and handling tasks.
  • Willingness to travel within Swindon and nearby areas.

Desirable

  • NVQ Level 2 in Health & Social Care or a commitment to achieving it.
  • Experience in Reablement, Home Care, Domiciliary Care, or Adult Social Care.
  • Training in Moving & Handling.
  • Familiarity with safeguarding and adult care services.
