An exciting opportunity to be a Referral and Assessment Coordinator in the Housing Team.
Salary: £28,000 per annum pro rata
Hours: 22.5 hours
Job Type: Fixed Term – 6 months
Location: Balham

About The Role
The Referral and Assessment Coordinator position is to provide excellent administrative support to the Referral and Assessment Lead by monitoring and processing enquiries and referrals daily. This includes coordinating referral and assessment documentation, recording activity accurately, keeping records up to date, and supporting updates on empty rooms.
Duties will include:
- Receive, process and record referrals from a range of sources, including local authorities, healthcare professionals, customers, families, and partner agencies via email and telephone.
- Process referrals by accessing the DPS platforms, Careplace and other referral sources.
- Support the Referral & Assessment Lead in scheduling assessments and reviewing appointments for customers with managers and external support services and agencies.
- Maintain assessment diaries and coordinate appointment arrangements.
- Coordinate the weekly circulation of the empty rooms spreadsheet by email to operational managers and teams for current information on new empty rooms and updates.
- Prepare general updates for systems, check for accuracy, and follow up queries with staff teams.

About You
To be a Referral and Assessment Coordinator, the following are essential:
- Experience of providing administrative support within a busy office environment.
- Experience of maintaining accurate records and databases.
- Understanding of confidentiality and data protection requirements.
- Understanding of effective customer service principles.
- Understanding of housing, care, support, health, or social care services.
