We're looking for an experienced Referrals & Development Lead to lead the growth and development of our supported living services across Moorview Care.
If you're passionate about creating new opportunities for people with learning disabilities, autistic people, and individuals with complex needs, and have experience managing referrals, assessments, and service development, we'd love to hear from you.
This is an exciting opportunity to play a key role in expanding our services, building strong partnerships with commissioners and stakeholders, and ensuring every person joining Moorview Care experiences a safe, person-centred transition.

About the Role
As our Referrals & Development Lead, you will lead the end-to-end referral journey, from initial enquiry through to assessment, transition, and successful placement within our services.
Working closely with operational managers, clinical professionals, commissioners, and families, you'll ensure every transition is safe, well-planned, and tailored to the individual. Alongside leading referrals, you'll play a key role in identifying new service opportunities, supporting business growth, and helping shape the future of Moorview Care.

This is a varied role that combines relationship management, operational planning, and strategic development, making it ideal for someone who enjoys building partnerships, solving complex challenges, and driving positive change.
Key Responsibilities
Referrals & Transitions
- Lead the referral process from enquiry through to placement.
- Coordinate assessments, environmental visits, and transition planning.
- Work collaboratively with operational and clinical teams to ensure medication, behavioural support, and risk management needs are fully considered.
- Support individuals and families throughout the transition process and early review period.
Service Development
- Support the development and opening of new supported living services.
- Contribute to Moorview Care's growth strategy and future service development.
- Complete feasibility assessments and develop business cases for new opportunities.
- Ensure new developments meet CQC, NICE, and organisational standards.
Partnership Working
- Develop and maintain strong relationships with Local Authorities, Integrated Care Boards (ICBs), housing providers, and other stakeholders.
- Represent Moorview Care at referral meetings, provider forums, and funding panels.
- Build collaborative partnerships that support sustainable service growth and positive outcomes.
Quality, Compliance & Governance
- Ensure referrals, assessments, and transitions meet legislative, safeguarding, and regulatory requirements.
- Support the development of policies, procedures, and organisational projects.
- Contribute to quality improvement, governance, and risk management activities.
Operational Support
- Work with operational teams to support recruitment, workforce planning, and resource allocation for new services.
- Monitor key performance indicators and provide regular progress reports.
- Support senior leaders with strategic initiatives and deputise where appropriate.

About You
You'll be a confident relationship builder with a passion for delivering person-centred services and developing opportunities that improve people's lives.
You'll bring:
- Significant experience within Health and Social Care, particularly referrals, placements, or service development.
- Experience working with commissioners, multidisciplinary teams, and funding panels.
- Strong knowledge of supported living services, complex transitions, and person-centred care.
- Excellent organisational, planning, and project management skills.
- Strong communication and stakeholder engagement skills.
- Sound judgement and the ability to make confident decisions.
- A commitment to co-production, inclusion, and trauma-informed practice.
- Level 5 Health and Social Care qualification (or equivalent) desirable.
- Full UK driving licence and willingness to travel across our regions.
