Registered Dietitian – Multi-Facility
Orchard Post Acute & Oakwood Gardens Care Center are seeking an experienced and compassionate Registered Dietitian (RD) to join our team. This position will provide nutritional services and support to residents at both facilities, working closely with nursing, dietary services, physicians, and the interdisciplinary care team.
We are looking for a Registered Dietitian who is passionate about improving resident outcomes through individualized nutritional care and who enjoys working collaboratively in a skilled nursing and post-acute care environment.
Compensation
Pay Range: $42.00–$43.00 per hour, based on experience
Essential Duties And Responsibilities
- Provide Registered Dietitian services for residents at both Orchard Post Acute and Oakwood Gardens Care Center.
- Complete comprehensive nutritional assessments for residents upon admission and as required.
- Develop individualized nutrition interventions and recommendations based on resident needs, diagnoses, preferences, and physician orders.
- Monitor residents at nutritional risk, including residents experiencing weight loss, weight gain, wounds, altered diets, or other nutrition-related concerns.
- Review resident weights, laboratory results, food intake, hydration status, medications, and clinical documentation.
- Participate in interdisciplinary care plan meetings and provide recommendations regarding residents' nutritional needs.
- Develop and update resident nutritional care plans.
- Evaluate residents receiving enteral nutrition and make recommendations as appropriate.
- Collaborate with nursing, physicians, dietary staff, speech therapy, and other members of the interdisciplinary team.
- Review therapeutic and mechanically altered diets for accuracy and appropriateness.
- Provide education to residents and families regarding dietary restrictions, nutrition goals, and therapeutic diets.
- Provide education and training to dietary and clinical staff as needed.
- Complete timely and accurate documentation in the electronic health record.
- Conduct kitchen and food-service observations to support compliance with sanitation, food safety, and nutritional standards.
- Assist the Dietary Services Department with menu review and nutritional adequacy.
- Participate in Quality Assurance and Performance Improvement initiatives related to nutrition and dietary services.
- Assist with survey preparation and respond to nutrition-related questions during regulatory surveys.
- Maintain confidentiality of all resident and facility information.
- Ensure compliance with federal, state, and facility regulations governing skilled nursing nutritional services.
- Perform additional duties related to nutritional services as assigned.

Qualifications
- Bachelor's degree in Nutrition, Dietetics, or a related field required.
- Current registration as a Registered Dietitian (RD/RDN) through the Commission on Dietetic Registration required.
- Must meet all applicable state and federal requirements to practice as a Registered Dietitian.
- Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute care preferred.
- Knowledge of Medicare, Medicaid, CMS, and skilled nursing nutritional requirements preferred.
- Strong knowledge of therapeutic diets, nutritional assessments, weight management, wound nutrition, and enteral feeding.
- Excellent organizational, communication, and documentation skills.
- Ability to effectively manage responsibilities between two facilities.
- Strong interpersonal skills and the ability to collaborate effectively with residents, families, and interdisciplinary healthcare teams.
- Proficiency with electronic health records and basic computer applications.
