What You'll Do

  • Conduct comprehensive nursing assessments and develop individualized plans of care.
  • Provide skilled nursing services based on each patient's needs and physician orders.
  • Monitor patient conditions, identify changes in health status, and communicate concerns appropriately.
  • Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence.
  • Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team.
  • Manage and prioritize an assigned patient caseload while maintaining high standards of care.
  • Complete timely, accurate, and compliant clinical documentation.
  • Participate in care coordination and help ensure positive patient outcomes.

What We're Looking For

  • Current Registered Nurse (RN) license in the applicable state.
  • Home health experience preferred.
  • Strong clinical assessment, critical-thinking, and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while collaborating effectively with a care team.
  • Strong organization and time-management skills.
  • Valid driver's license and reliable transportation.
