What You'll Do
- Conduct comprehensive nursing assessments and develop individualized plans of care.
- Provide skilled nursing services based on each patient's needs and physician orders.
- Monitor patient conditions, identify changes in health status, and communicate concerns appropriately.
- Educate patients and caregivers on medications, disease management, treatment plans, and strategies for maintaining health and independence.
- Collaborate with physicians, therapists, care coordinators, and other members of the interdisciplinary team.
- Manage and prioritize an assigned patient caseload while maintaining high standards of care.
- Complete timely, accurate, and compliant clinical documentation.
- Participate in care coordination and help ensure positive patient outcomes.
What We're Looking For
- Current Registered Nurse (RN) license in the applicable state.
- Home health experience preferred.
- Strong clinical assessment, critical-thinking, and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work independently while collaborating effectively with a care team.
- Strong organization and time-management skills.
- Valid driver's license and reliable transportation.
