Position Summary: The Home Care Services Liaison is responsible for building and maintaining relationships with community referral sources. This role serves as a key link between the agency and the community, ensuring awareness of services, supporting growth initiatives, and ensuring smooth transitions for patients onto home care and hospice services.

Key Responsibilities:
- Build and maintain strong relationships with hospitals, medical practices, SNFs, ALFs, and other referral sources
- Educate referral sources on available home care and hospice services and admission criteria
- Generate referrals through ongoing outreach, networking, and relationship management
- Conduct regular in-person visits and follow-ups with referral sources
- Collaborate with intake and home care/hospice leadership to ensure smooth patient transitions and admission to services
- Represent the agency at community events, health fairs, and meetings
- Stay informed on industry trends, regulatory changes, and competitor activity
Qualifications:
- Active, unencumbered Registered Nurse License in the state of practice
- Minimum 1–2 years of clinical nursing experience
- Experience in home health, hospice, case management, or intake coordination preferred
- Knowledge of Medicare/Medicaid regulations for home care and hospice
- Strong assessment, organizational, and communication skills
- Proficiency with electronic medical records (EMR/EHR) systems

Work Environment: Primarily field-based with travel within service area. Occasional office work for meetings and reporting. Flexible schedule may be required to attend community events.
