Salary range: Grade: 10 £43,680 - £45,750 p.a. plus fantastic benefits Contract: Permanent Hours of work: 36 hours per week Location: Croydon BWH

If you are a qualified Social Worker registered with Social Work England and have a minimum of two years' post-qualification experience, including completion of your ASYE, we would love to hear from you. This is an exciting opportunity to join our dedicated Older People's Services team in Croydon, where you will play a key role in supporting older adults to maximise their independence, wellbeing, and quality of life. Working within a strengths-based, person-centred framework, you will be part of an innovative and supportive team committed to delivering excellent outcomes for residents and their families.

About the Team

Our Older Adults Team consists of:

  • Health and Wellbeing Assessors
  • Social Workers
  • Experienced Social Workers
  • Advanced Social Workers
  • Team manager

The team works closely with health professionals, Integrated Care Networks, voluntary and community sector organisations, hospitals, and care providers to deliver coordinated, person-centred support for older adults. You will work with individuals experiencing a wide range of needs, including frailty, dementia, mental health challenges, physical disabilities, safeguarding concerns, and complex care and support requirements.

Through effective partnership working, you will play a key role in promoting independence, reducing risk, and improving outcomes for older adults, ensuring they receive the right support at the right time to live safe, fulfilling, and meaningful lives.

About the Role

Key responsibilities include:

  • Build positive and trusting relationships with residents, families, carers, and community partners.
  • Use Community Led Support principles and other strengths-based approaches to hold meaningful conversations that focus on people's aspirations, strengths, gifts, and abilities.
  • Meet people in locations where they feel most comfortable, including community venues and local neighbourhood settings.
  • Support individuals to identify and access community resources, networks, and opportunities that promote independence and wellbeing.
  • Connect people with local groups, services, and community assets to reduce isolation and strengthen natural support networks.
  • Work collaboratively with individuals and those important to them to co-produce personalised support plans that reflect their goals, strengths, and desired outcomes.
  • Promote prevention and early intervention by identifying solutions that build on individual and community strengths before the need for statutory intervention arises.
  • Undertake strengths-based assessments and support planning in line with relevant legislation and organisational policies.
  • For individuals with eligible care and support needs under the Care Act 2014, arrange and coordinate appropriate statutory and commissioned services where required.
  • Ensure that any formal support complements and enhances the strengths and contributions of families, friends, carers, and community networks.
  • Maintain accurate records and contribute to effective multidisciplinary and partnership working across health, social care, voluntary, and community sectors.

To be successful in this role, you will have:

  • A recognised Social Work qualification, with minimum of 2 years post qualification /ASYE experience.
  • Enhanced DBS check.
  • Current registration with Social Work England.
  • A sound understanding of adult social care legislation and policy, including the Care Act 2014, Mental Capacity Act 2005, Mental Health Act, and relevant safeguarding frameworks.
  • Experience of working with older adults and individuals with care and support needs within a health, social care, or community setting.
  • The ability to develop and maintain effective relationships with individuals, families, carers, colleagues, and partner organisations.
  • Strong assessment, decision-making, and problem-solving skills, with the ability to manage complex situations confidently and professionally.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with a wide range of people and stakeholders.
  • A commitment to strengths-based, person-centred practice that promotes choice, control, independence, and positive outcomes.
  • The ability to work collaboratively across multidisciplinary teams and community networks to deliver integrated and coordinated support.
  • A commitment to continuous professional development and reflective practice.
