The post holder will provide high-quality, person-centred nursing care, ensuring individuals receive respectful, holistic support tailored to their needs. Working across all five Health Board localities, they will help reduce health inequalities by engaging with hard-to-reach groups, improving access to services, and addressing barriers to care.
The role includes promoting understanding of the social determinants of health through education, advice, and support for communities and multidisciplinary teams. Working collaboratively with partner organisations, the successful candidate will contribute to innovative approaches that improve health equity and advocate for vulnerable individuals.
As an autonomous practitioner, they will be responsible for delivering evidence-based care, using advanced clinical skills, critical thinking, and professional judgement to support complex decision-making.

This post is a fixed term/secondment for 11 months from start date due to secondment cover. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

Main duties of the job
The post holder supports the Senior Nurse in the delivery and development of high-quality healthcare services for socially vulnerable, underserved, and hard-to-reach populations across all five Health Board localities. The role contributes to the management and coordination of services, ensuring safe, effective, and equitable care while reducing health inequalities. The post holder works collaboratively with multidisciplinary and multi-agency teams, deputises for nursing colleagues when required, and represents Primary and Community Care at operational and strategic levels. Acting as a clinical expert and professional role model, they promote evidence-based practice, quality improvement, and the implementation of healthcare policies and procedures.
Key responsibilities include undertaking initial health assessments for asylum seekers, completing the All Wales Health Assessment Record (Blue Book), delivering cultural awareness training to healthcare professionals, and supporting integrated care pathways through effective assessment, planning, implementation, and evaluation of services. The role also involves educating professionals and communities on the health needs and social determinants affecting vulnerable populations to improve health outcomes and service accessibility.
