Band 3 Rehabilitation Assistant
An opportunity has arisen for a Full-time Rehabilitation Assistant to join our Short Term rehabilitation service who are motivated to provide rehabilitation to help patients improve their abilities and achieve their rehabilitation goals, through therapy led care.
Rehabilitation Assistants work as part of the multidisciplinary team, working closely with physiotherapists and occupational therapists. We provide rehabilitation in the patients’ home environment with a focus on promoting independence, meeting rehabilitation goals and reducing care needs in the community.
This is an exciting opportunity for a health care support worker who already has some experience of working with patients in an acute or community healthcare setting to expand their skills and work within a friendly community team.
Ealing Community Partners is a group of NHS, Ealing Council and voluntary organisations working together to deliver community health and care services for the people of Ealing.
The post holder will need to be flexible to meet the needs of the Service and it is essential they have a current driving license and access to a vehicle as extensive travel across the Borough is required.
Main duties of the job
- To work as a Rehabilitation Assistant within a multidisciplinary Short Term Rehab team, providing rehabilitation and or supported discharge to people in their own home or place of residence and/or designated buildings.
- To work under the guidance of health care professionals to assist therapists by carrying out delegated aspects of patient care without direct supervision and reporting back patient’s progress or issues to the therapist.
- To be responsible for ordering equipment and department supplies.
- To undertake specific skilled support work to aid the rehabilitation of patients in the community.
- To run designated Mobility Clinic sessions, providing mobility aids, undertaking equipment fitting and adjustments, and ensuring patients are competent and safe in their use.

Short Term Rehabilitation Service is a very friendly team in Ealing. We work together to provide excellent rehabilitation to our patients and provide a supportive and flexible environment for team members.
We are part of the Maximising Independence service line within Ealing Community Partners (ECP).
The team are supported in their professional development with regular training and clinical supervision ad opportunities to expand your clinical skills.
Detailed Job Description And Main Responsibilities
- To provide physiotherapy and occupational therapy intervention as part of a specific treatment plan agreed and documented by physiotherapist/occupational therapist.
- To undertake tasks in delivering patient care, rehabilitation or social care as directed by the service professionals in a range of Short-Term Rehab settings.
- To manage the rehabilitation of patients as delegated by the physiotherapist or occupational therapist.
- To work independently in the designated clinical area/patients home following an agreed treatment plan.
- To instruct patients and carers, providing direction and guidance on a range of physiotherapy or occupational therapy and mobility, functional activities, as directed by the physiotherapist or occupational therapist.
- To undertake skilled support work with patients individually or in a group with or without supervision of a physiotherapist or occupational therapist.
- To follow departmental and Trust guidelines for obtaining consent and use diplomacy and gentle persuasion to enhance patient’s compliance in treatment programmes.
- To work with family members, carers and support workers to enable them to carry out treatment programmes planned by the physiotherapist or occupational therapist.
- Manage own workload allocated by the physiotherapist or occupational therapist.
- To work as a member of the multidisciplinary team and liaise with the team effectively to ensure co-ordinated approach to patient care and attend meetings as delegated by the physiotherapist or occupational therapist.
- Maintain comprehensive and accurate records of work undertaken, including documenting in line with Trust and service standards.
- To attend in-service training, clinical supervision and team meetings as required.
- To assist physiotherapist or occupational therapist with complex patients in their own home environment.
- Have the ability to handle patients with mobility problems, in an enabling way. This may include the use of hoists, sliding sheets, wheelchairs, mobility aids etc.
- To participate in a rota to work over a 7-day week (i.e. including weekends) when required by the service.

Education and Qualification
Essential criteria
- Good level of general education (GCSE Maths and English)
- Evidence of continuous development
Desirable criteria
- NVQ Level 3 qualification in health and social care, or equivalent experience
Knowledge and Experience
Essential criteria
- Have knowledge and understanding of the role of Rehabilitation professionals in the community
- Previous work experience, voluntary or paid, as a Rehabilitation assistant or similar Health or Care Assistant role in a social or healthcare environment.
- Experience of working with older people and people with long-term conditions
- Able to work in a team
Desirable criteria
- Understanding the NHS and Social Care
- Understanding the roles of physiotherapy and occupational therapy
Skills And Abilities
Essential criteria
- Able to communicate clearly and accurately, verbally and in writing, to a wide range of people including users, colleagues, carers, and managers.
- Basic IT skills – able to use a computer for emails, word & general internet use.
- Ability to work in a team, demonstrate flexibility and respond to changing demands and priorities.
- Able to drive and have use of a car for work or similar

- Ability to prioritise tasks and organise time effectively.
