Remote Rehabilitation Case Manager

£37,000–£40,000 + £4,200 Car Allowance + Annual Bonus

Full-Time (37.5 hrs) | West Midlands

Sentra Care is delighted to be partnering with one of the UK’s largest and most reputable Case Management companies to welcome an experienced Rehabilitation Case Manager to their growing team.

This is a fantastic opportunity for a Nurse, Physiotherapist, Occupational Therapist, who is passionate about supporting individuals through injury, and recovery. You’ll play a key role in helping clients regain independence, return to function, and achieve the best possible outcomes.

What You’ll Be Doing:

  • Carry out Initial Needs Assessments and manage a caseload of 20–35 clients
  • Identify barriers to recovery and design tailored rehabilitation plans
  • Coordinate treatment, liaise with clinicians, and build strong relationships with all providers
  • Maintain regular client contact via visits, phone, video, and written updates
  • Produce high-quality reports and accurate case management records
  • Arrange or undertake specialist assessments as needed
  • Working remotely with some field based community assessment and meetings required

What We’re Looking For:

  • A degree in Nursing, Occupational therapy, Physiotherapy - NMC/HCPC registration
  • MUST have experience in Brain Injury, Spinal Injury, Orthopaedics and/or Trauma Rehab
  • Strong communication, clinical reasoning, and decision-making skills
  • Someone confident working independently and managing their own caseload
  • Comfortable using case management systems and Microsoft Office
  • Must be a car driver
  • We have live locations in Bristol, London, West Midlands & Lincolnshire - Must be in or around these locations
  • Must have the right to work in the UK - sponsorship for a skilled visa isn’t available
