Job Overview
An exciting opportunity has arisen in renal services to work as a Chronic Kidney Disease Nurse Specialist at Wrexham Maelor Renal Unit. In line with current research and best practice the post holder will focus on the prevention of the progression of chronic kidney disease by working closely with the Consultant nephrologist, primary care and other members of the multidisciplinary team. Based in secondary care, the post holder will be expected to assess, plan, implement and evaluate holistic care for patients with Chronic Kidney Disease (CKD) stages 3-5 working alongside the consultant nephrologist in the participation of long term planning for the future. The CKD nurse will specialise in blood pressure and anaemia management, whilst supporting the patient with education and counselling in pre dialysis education, transplantation and end of life care. Main Duties of the Job
JOB SUMMARY / JOB PURPOSE:
Prevention of the progression of CKD is paramount to the future management of renal services and the emphasis should be on pre dialysis care and transplantation. Research has demonstrated that good pre dialysis management will slow down the descent on to renal replacement therapies thus reducing the burden of secondary care services.

- In conjunction with Consultant Nephrologists, Primary Care colleagues and other members of the renal multidisciplinary team, the post holder will assess, plan implement and evaluate holistic care for patients with chronic kidney disease (CKD) stages 3-5.
- The post holder will be based in secondary care but will be expected to work closely with primary care, providing expert advice and training to both patients and health care professionals.
- An important aspect of this role is increasing the skills of non-secondary care health professionals within the area of prevention and shared management of CKD progression.
- The aim will be to develop a clinical model for the management of chronic kidney disease patients through collaborative working.
- The focus is on creating a continuum of care which includes relevant professional colleagues within the Trust and the community working across professional and organisational boundaries.
